Illawarra Children's Services approves Inclusion Support Subsidy (ISS) applications and processes ISS Claims for Payment. The following forms are available for services to assist with submitting claims for payment:

Please note Inclusion Support Facilitators (ISFs) are not involved in the processing approving of ISS applications and claims for payment. They assist services to develop Service Support Plans and complete the Inclusion Support Subsidy application form, as required.
